If unknown, please schedule an appointment with the Utility Department.
(Do not return the survey marked “Unknown.”)
If non-lead, please indicate the specific material: Galvanized Steel, Copper, Plastic, or CPVC.
(Do not return the survey marked only “Non-lead.”)
Ways to return your information:
-Mail the survey back
-Drop it off in the City’s drop box
